For my church, I wanted a traditional bulletin style format baby dedication certificate – a classic design template that could be used consistently for each child dedication service at our church. I also wanted to customize them with our information (aka if the grandparents presented a child) or change the scripture verse. After searching around for some online templates, I put together this simplified keepsake to give parents after the child blessing ceremony. Use the web download links above to customize this for your church.
Baby Dedication (Template)
“Let the children come to me; do not hinder them, for to such belongs the kingdom of God.”
Mark 10:14
This is to certify that ____________________ (enter child’s name)
was publicly dedicated to the Lord on the ____________ (enter day) day of ___________ (enter month) in the year of our Lord __________ (enter year)
at _______________________________ (enter church’s name)
Parents (parents name)
Pastor (pastor name)
You can customize everything about this template document if you choose the Microsoft Word doc version. You can add or change the scripture reference. Type in the child, parents, church name and pastors name before printing. You could even insert a photograph of the baby or the actual dedication service. The PDF version has fillable blanks, but those can be edited directly from the custom doc file format.
Just be sure to print these on baby dedication certificates or heavy resume style stationary. You should download and print the document 2-sided and then fold into this church bulletin format.
This is an important keepsake that families will cherish as their children grow in the Lord. I’ve also left space for the pastor to write his own short note or prayer under the Bible verse on the inside flap. This extra touch will mean even more in years to come. Even if the child attends another church in the future, this will remind them of the spiritual connection to your congregation.

There are some other baby dedication certificates available on the internet that can be downloaded and printed. We couldn’t find one we liked (most were trying to sell a printed version). That’s why we created our own.
The exact meaning depends on the church. In many congregations, they do not baptize children but have a ceremony of child / parent dedication. It’s a way to formally recognize the blessing of children and for the parents to commit publicly to sharing the Gospel with the coming generation.
Child & parent dedication is much more than a prayer time and handing out baby dedication certificates.
